Medical Social Services
We offer comprehensive counseling and support to patients and their families, providing guidance through a difficult process that can take an emotional, financial, and spiritual toll.
We understand that the end of life can be an incredibly difficult time. That's why our care team is committed to providing compassionate medical, spiritual, and bereavement counseling for both you and your family - so that everyone involved feels supported. Additionally, our experienced staff is available to assist with whatever tasks may make things a little easier throughout this process - helping families come together as they navigate through these challenging times.

Our team is here to help every step of the way - from ensuring all necessary documents are in order and coordinating daily medical needs to providing emotional support for everyone involved. We assign an experienced social worker who will visit each new hospice patient within five days and continue visiting on a regular basis if needed.

  • Advanced Directives (Living Wills, Power of Attorney (POA), and Do-Not-Resuscitate (DNR) orders).

  • In cases of increased care needs, we can assist with finding a placement in a nursing facility or care home.

The conversation project

When it comes to end-of-life care, so many of us know how tough the conversations can be. But what if there was a way for you and your family to talk about your wishes openly? Enter The Conversation Project – an incredible website dedicated to helping everyone get past that uncomfortable wall we often face when discussing our final days. With guidance from this project's resources, people everywhere are encouraging real transformation in culture through open dialogue instead of avoidance tactics - meaning families don't have to be left with guilt or uncertainty after losing someone they love.

 

The Conversation Project provides an incredible opportunity to make sure your voice is heard - even during difficult times. By downloading the starter kit, you can start discussing and documenting what kind of care matters most to you (or those close to you). It's a powerful way for everyone involved to feel empowered in their end-of-life conversations.

Advanced Directives including Living Wills, Power of Attorney (POA), and Do-Not-Resuscitate (DNR) orders.  Our Medical Social Workers provide emotional support to the patient, family, and staff, and provide ongoing education and assistance with available Hummingbird Hospice services. One of our Medical Social Workers will make an initial visit to a new hospice patient within 5 days of admission and will continue to make visits on a monthly basis, or more frequently as needed.
