Are You Hospice Eligible?
We understand that obtaining your medical records is essential. We'll do our best to process your request within 5 to 7 business days, but HIPAA regulations allow up to 30 days. Please keep in mind that the time it takes may be influenced by factors such as the size of your record, the availability of the documents, and the volume of requests we receive
You can collect your records during our regular business hours, Monday through Friday, from 9:00 a.m. to 5:00 p.m., except for federal holidays. Alternatively, we can arrange a special appointment. Thank you for your patience and trust.
To obtain your records, simply submit a completed Medical Records Release of Health Information Authorization Form. Please note: there may be a charge for reproduction costs.
To ensure the privacy and security of your information, photo identification is required prior to releasing records. If you need someone else to pick up your records, simply give us written permission and they must also bring photo identification.
